A tummy tuck (abdominoplasty) is a procedure that removes excess fold of abdominal skin and fat and tightens the abdominal muscles. the net effect of the procedure is a complete restoration of a youthful abdominal contour. This patient had a heavy abdominal contour with sagging of the abdominal and pelvic tissues. Her abdominal wall was lax and rounded. She was treated with a full abdominoplasty and liposuction of the flanks. The scar is kept very low on the pelvis, and is well concealed with the smallest string bikini, as depicted in the post-operative photo. This post operative photo was taken just 6 weeks after surgery. This patient had a swift and comfortable recovery.

Fat grafting to breast for reconstruction found to be safe in controlled clinical trial

An academic paper was published recently in the medical journal Annals of Oncology documenting an important safety factor for the fat grafting procedure. The full journal article can be read here

One criticism of fat grafting (referred to as ‘lipofilling’ in the paper) to the breast for both reconstructive and cosmetic purposes has been the behavior of the transplanted fat derived stem cells. There has been concern that stem cells transplanted into the breast might ‘turn on’ cellular signals that make otherwise normal breast tissue turn into cancer. This concern was raised many years ago, and is part of the basis for the American Society of Plastic Surgery’s hesitancy to support the procedure (until very recently). However, clinical papers published within the past two years that have looked at large numbers of patients that have had fat grafting to the breast have not supported these, and other concerns about fat grafting to the breast.

This recent paper, out of the European Institute of Oncology, fails to show an increase in breast cancer recurrence amongst women that have had breast cancer procedures such as lumpectomy, quadrentectomy and mastectomy and also had breast reconstruction using fat grafting techniques.  This case-control clinical study compared the fat grafting group to a large cohort of breast cancer patients that received breast cancer surgery without a fat grafting reconstructive procedure.

The natural history of invasive breast cancer treated with all type of breast cancer surgery is a local recurrence rate of about 5%. This study fails to show an increase in local recurrence rates of invasive breast cancer amongst women who, on average, were 56 months (4.6 years) post-op from cancer surgery and 26 months (2.1 years) post-op from fat grafting. This is encouraging news !!

Of course, longer term follow-up studies are needed (5 years, 10 years, 15 years). At least for the short and medium terms, fat grafting to the breast does not appear to result in increased local recurrence rates for invasive breast cancer who undergo fat grafting as part of their breast reconstruction procedure.

We offer fat grafting to the breast for cosmetic and reconstructive purposes, and will continue to do so, based on encouraging clinical results such as this one.

I read with interest the searing clinical article on fat grafting to the breast in the April 2011 edition of the Journal of The American Society of  Plastic and Reconstructive Surgery (PRS, for short…) authored by a group of Chinese plastic surgeons.

The Chinese fat grafting article slams the practice of fat grafting to the breast. Wang, et al conclude that radiologists in their practice could not discern between calcifications on a ‘digitized’ mammogram that are a result of fat grafting to the breast, and calcifications that might represent breast cancer. The group recommended breast biopsy (a surgical procedure)  in a number of these patients, with the result of  ’fat necrosis’ (no surprise !), which is not breast cancer.  At the end of their paper, they make the illogical conclusion that

‘ The mammographic confusion constitutes the problem rather than the success of the procedure itself, and the method should continue to be prohibited.’

This paper echoes the concerns raised several years ago regarding the managment of calcifications noted on mammography of patients that have had fat grafting to the breast. However, in recent years, digital mammography and contrast MRI have become much more accurate in descerning the difference between calcified bits of  fat necrosis and malignant micro-calicifcations. In fact, numerous papers in the radiology literature have confirmed that calcified fat necrosis and malignant microcalcifications can be easily distinguished with modern digital mamographic equipment and experienced radiologist reviewing the images. Other clinical studies that have followed fat grafting patients long term note no increase incidence in breast malignancy in this patient population.

In my own practice of fat grafting to the breast, we obtain digital mammography on all patients pre and 6 months post fat grafting, and have encountered no concerning calcifications.

In summary, this recent retrospective review documents nothing more than a Chinese plastic surgical group’s aggressive management of what appear to be benign micro-calcifications. In contrast, fat grafting surgeons world-wide have been buyoed by the numerous recent publications in PRS and other journals that support the safety and efficacy of fat grafting to the breast. Every other surgical procedure done to the breast induces some fat necrosis and calcification that can show up on mammogram. Yet no one is calling for the ban on breast reduction, mastopexy or augmentation !

Fat grafting to the breast is yet another tool that plastic surgeons can begin to use to improve form and restore volume to the breast.
